Congregate Dining and Social Programs

You or your loved one can enjoy a delicious meal, entertainment and time with friends [both new and old].  Each dining experience offers something a little unique in a different location in Perth East and Atwood.  There are opportunities for cards, games, musical entertainment and speakers.  They also vary between:  brunch, lunch, supper and Barbeque meals.  Some programs offer a take-out option.

Friendly Visiting

If you live alone, feel lonely, isolated or could just use a friend, this program may be an option that is right for you.  This program will provide you with the support of a trained volunteer who will meet on a one to one basis and visit in your own home.  These regular visits can include a telephone conversation, or enjoying activities outside the home.

Eligibility – Those over 65 who live alone with no regular contact.  No respite care.

This service is available in Perth East.

This program is dependent on volunteer availability and client location.

There may be a wait list as the program is in high demand.

Fee – Free

Volunteers CAN:

  • Provide companionship in the client’s home
  • Provide companionship over the phone
  • Provide companionship at a mutually agreeable place: coffee shop, park, library, etc

Volunteers CANNOT:

  • Do medication reminders
  • Complete any personal care tasks
  • Complete any housekeeping tasks
  • Drive the client in their car
  • Drive the client in the client’s car

Lifeline – Emergency Response System

Community Outreach provides the Lifeline Emergency Response System to people within Perth County and area through our partnership with Lifeline.

Lifeline provides both you and your family peace of mind and allows you to live at home independently.

Lifeline is an easy-to-use personal response system that lets you summon help any time of day or night.  It is as simple as pressing a button to get connected to a Personal Response Associate who will ensure you get help fast.

Lifeline can benefit those who are at risk of falls or medical issues, living alone, at risk of readmission, or use assistive devices for walking

Community Outreach would be happy to make a referral on your behalf.

Health Screening/Clinics

Community Outreach is able to provide a variety of Health Screening Clinics to the community as a result of the wonderful partnerships that have been formed and a shared value on the benefits of offering things locally.

Our health screening clinics will help you to monitor and maintain or improve your health.

Blood Pressure Clinics

It is important for people to know what their blood pressure is and/to monitor it on a regular basis. Monitoring your blood pressure for changes can be preventative and or indicate the need for medication.  It can also be an early indicator for heart and stroke disease.

You can have your blood pressure checked for free at any drop-in blood pressure clinics:

Mini Mart in Milverton – 3rd Thursday of every month 9am- 11am
Milverton Place in Milverton – 3rd Month of every month 9:15am

Trained staff and volunteers measure your blood pressure and record it on a card for you.
Free of charge.

Foot Care Clinics

Regular foot care helps to maintain healthy feet, and is especially beneficial for people with diabetes, arthritis or circulation problems. Your foot care visit will include an assessment of the condition of your feet, pedicure and treatment of basic foot conditions.
Foot care is done by a Registered Nurse with Advanced Foot Care qualifications
Appointments are required by calling 519-595-8755.
Fee: $45.00 [cash] New clients $55.00 [cash] per appointment

Diabetic Clinics

If you are diabetic, attending this diabetic clinic will help you manage your condition better.
You will have the opportunity to have your blood sugar level checked.
Speak with Registered Nurse, the Pharmacist and a Dietician to gain insight into your diabetes Provide suggestions for changes that can be made to help you manage.
Offered at Village Pharmacy yearly
Free of charge.
This service is offered in partnership with the Village Pharmacy, Huron Perth Healthcare Alliance – Stratford site and Community Outreach Services

Hearing Screening

Hear Well Be Well

Being able to hear well certainly makes life easier and more enjoyable. Regular hearing checks can help you monitor your hearing and to identify early signs of hearing loss.  They can also help with adjusting and cleaning your hearing aids.
Free of charge

A Registered Audiologist from Hear Well Be Well provides this service.

Lifelabs Clinics

At the Lifelabs clinic, you can have your blood work completed

Exercise and Falls Prevention

Feeling good, building strength, maintaining balance and improving functional abilities are all excellent reasons to participate in any of the exercise programs offered.  They will also help to reduce the risk of falls and improve your quality of life.

These classes are geared to older adults and are offered at different levels to complement your ability.  They are offered at a variety of locations 2-5 times per week.

Classes are led by fitness instructors who have been certified through the Canadian Centre fore Activity and Aging Senior’s Fitness Certification program.

Retirement Home Falls Prevention – Level 1

Classes are offered in retirement homes.  The class is designed to maintain and support improvement for those with limited mobility.  The classes are primarily seated with standing components as well.  Gait aids are welcome.

Group Fitness Gentle – Level 2

Level two classes are offered in community settings.  The class is designed for those with a moderate level of ability.  This gentler version of our Group fitness class is great for individuals with limited mobility. Achieve your goal of walking for short durations by completing balance, strength and cardio exercises while seated or standing.
Participants with higher levels of mobility are welcome.

Group Fitness – Level 3

Level 3 classes are offered in community settings. The classes are designed for those with a moderate to vigorous level of ability, providing the option to participate at a higher level of intensity. This class includes balance, strength and cardio exercises.  Learn how to improve your stability and prevent falls.

Seniors Fitness Testing

When you participate in fitness programs you are given an opportunity to assess your functional fitness levels.  These assessments meet scientific standards for relativity and validity.  The process will confirm your functional fitness abilities regarding, cardiovascular, leg strength, upper body strength, balance and flexibility.  Assessments are done twice a year to help you set your goals and to see your achievements. You are provided with a printed copy of your results and recommendations.

Assessments are FREE.

In- Home Exercise Program

Instructors will come to your home and guide you through 10 simple and progressive exercise specifically designed to keep you living independently and prevent falls.

Offered once per week for a 10 week session
There is a small fee for each visit

Urban Poling

Give walking a new twist by using the urban poles to enhance the exercise benefits of walking. Training sessions are available. Once trained you are ready to go either with a group or on your own. Poles can be rented or purchased through the Community Outreach office.

In Door Walking Clubs

Offered at Perth East Recreation Centre, Monday to Friday from 9 am – 11:30 pm

Offered at Milverton Place [tenant program] daily

Helping to keep you both active and safe while you walk in a flat, dry and safe walking environment.

Registration and guidance is provided to all new people.

YOGA

Offered at St. Peters Lutheran Church in Milverton.

Fridays from 10:00am to 11:00am

Lead by a certified instructor.
